Incident Reporting
Incident reporting is a crucial part of maintaining a safe and secure environment for everyone in our community, and it’s important to report these incidents promptly. This page is here to guide you through the different types of incident reports, helping you determine which one you need to file. From safety and security concerns to whistleblowing and data issues, you’ll find the right form here to ensure your report is handled appropriately and efficiently.

Medical Incident
Examples include:

Injury while performing duties for North Way (staff or volunteer)

Injury while participating in a North Way event/ministry

Slip & Trip/Fall (regardless if injury occurred)

Vehicle accident (either NW vehicle, involved the property)

Anytime Child/Youth Services is contacted
-
EMT services were called
-
Any situation that risked the safety of a child in NW's care (elopement/missing child, safety policy violation, etc.)
Security Incident
Examples include:

When someone is exhibiting suspicious activity/behavior that should be monitored (see here for more details)

Theft, property damage, or vandalism of church property
-
Verbal/written threats or harassment
-
Display/ use of a firearm on NW property
-
Individual in inappropriate or unapproved area (such as wandering in the Kids area)

Altercation between two individuals that required intervention

Local law enforcement were summoned
Speakup Report
Example:

This report provides a safe and confidential way to raise concerns about severe issues or non-compliance (such as unethical behavior, misuse of resources, or any other actions that go against North Way's values wherein an individual should be held accountable)
Cyber/Data/Privacy
Examples include:
-
Unintended/Inappropriate sharing of confidential data
-
North Way account is compromised (i.e. clicked on a phishing email, password was compromised, device is lost/stolen, etc.)*

Suspected/known unauthorized access to North Way data or systems*
